NEPA Flames
DISCLAIMERS AND WAIVERS - PARENTAL CONSENT GENERAL DISCLAIMER: Any and all decisions related to the operation of the club or the individual teams will be made, as appropriate, in the sole discretion of the coaches, club operators, and/or the appropriate committee chairmen. PARENTAL WAIVER/STATEMENT OF CONSENT: As a parent/legal guardian of a potential NEPA Flames basketball player, I realize the risk of injuries involved in the game of basketball, and agree to hold harmless the NEPA Basketball Assoc., along with their coaches and volunteer staff, for any and all injuries incurred by my daughter while participating in activities, tryouts, practices, games, or any other activity with the NEPA Basketball Assoc, including transportation to and from the activities. In addition, I agree not to pursue any claims for accident or injury against any other group or organization providing practice facilities or equipment for the NEPA Basketball Assoc. I understand that my daughter's membership in the Amateur Athletic Union may provide supplemental insurance for medical bills resulting from injuries caused in accidents during sanctioned practices or competitions of either organization. I further recognize that I must first file a claim with my daughter's primary health insurer to be eligible for benefits from the sanctioning bodies. I further agree that I will not allow my daughter to participate in any NEPA Basketball Assoc activity that directly conflicts with any school basketball event (game, practice, etc.) that she is required to attend. PARENT PARTICIPATION OPPORTUNITIES: NEPA Flames parents are openly encouraged to participate in our club.
